How to Get Started with Fertility Treatments at FIRM

To make a new patient appointment with our office, you will need to have the following information for you and your spouse/partner (if applicable):

  • Social Security Numbers
  • Legal Names
  • Addresses
  • Contact Telephone Numbers
  • Date of Birth
  • Insurance Company, ID, Numbers, and Provider Contact Numbers

When you have this information available, please call the number for the office you are interested in visiting and ask to make a new patient appointment.  After setting your appointment, you can download your new patient paperwork in the “Patient Resources” link under “Getting Started”, or click here.

Policies

Appointments

Appointments are made with the receptionist after each office visit. You are expected to keep your appointments and arrive on time. We recommend that you plan to arrive twenty minutes early to allow for parking. Should you find it necessary to cancel or change an appointment, please contact our office at least twenty-four hours ahead of the scheduled appointment time. There will be a fee charged for appointments missed or canceled with less than twenty-four hours notice.

Telephone Calls

You may reach the office staff by calling our main office number, 904-399-5620. You should call our office during normal business hours (Monday through Friday, 7:30 am to 12:30 p.m./1:00 pm to 4:00 pm), for routine questions such as those regarding your treatment plans, medications, insurance, or payment questions. Our main office number should also be utilized for changing or canceling your scheduled office appointment time.

After hours phone calls are for emergencies only. If you have an emergency after office hours, please call our main office number (904-399-5620). The answering service will put you in touch with either the office nurse on-call or the physician. In the event of a Life Threatening Emergency, Call 911. We ask that you please notify our office about the medical emergency situation as soon as possible.

Notifying the office of changes

Please notify the office immediately if you change your home or work telephone number, or if you change your address. It is important that you keep your contact information current so that our office can notify you in a timely manner of your lab test results or medication changes. If you have a health insurance plan change, please notify our office of the new health plan’s effective date.
