IVF Guarantee Program

The costs associated with in vitro fertilization are considerable and unfortunately a pregnancy cannot be assured.  This expenditure may prevent some from pursuing other family building options, e.g. donor egg in vitro fertilization or adoption.  For those patients whom this may be a concern, we offer the IVF Guarantee Program.

If a patient qualifies for the program, they are allowed up to three fresh IVF cycles as well as all cryo IVF cycles related to their fresh IVF cycles for a set fee.  If the couple does not become pregnant and successfully pass twenty weeks of gestation after three fresh IVF transfers, as well as the transfer of all their cryopreserved embryos, they will be refunded 70% of their $18,500 ($12,950).

The cost of the IVF Guarantee Program is $18,500. This fee includes:

  • Three fresh IVF cycles, and all frozen embryo transfer cycles, to be accomplished within a sixteen (16) month period after starting the first fresh IVF cycle
  • Monitoring ultrasounds and bloodwork
  • IVF egg retrieval
  • Embryology services including ICSI and assisted hatching, if required
  • Embryo cryopreservation and storage for the fifteen (15) months after the conclusion of the first fresh IVF cycle

The original cost of $18,500 does not include:

  • Medications
  • Anesthesia
  • Epididymal/testicular sperm extraction
  • Pre- or post- IVF anatomic and medical/laboratory assessments, or necessary surgical corrections
  • Monitoring done at an outside facility
  • Embryo biopsy or pre-implantation genetic diagnosis
  • Storage of embryos beyond the initial fifteen month period
  • Donor services (sperm and/or egg)
  • Pregnancy lab work and monitoring

The above items will be necessary for some couples and not for others.  Costs for these non-included procedures will be calculated at usual F.I.R.M. rates.  Medication costs will vary depending on the amount required and where they are purchased.

To qualify for this program the patient must:

  1. Not have insurance coverage for IVF
  2. Be 37 years of age or younger prior to her first IVF retrieval and must finish her last IVF retrieval by age 38
  3. Have a uterus capable of carrying a pregnancy to term (i.e. there cannot be significant pathology predisposing to poor implantation or miscarriage, unless corrected prior to treatment)
  4. Have two intact ovaries without significant pathology
  5. Have a body mass index (BMI) ≤ 33
  6. Have normal ovarian reserve evaluation
  7. Have a stimulated endometrial thickness greater than, or equal to, 7 mm
  8. Not have a communicating hydrosalpinx (tube filled with fluid)
  9. Not have had more than one prior failed IVF cycle
  10. Not have any medical condition predisposing to poor implantation or miscarriage
  11. Not have a history of recurrent pregnancy loss
  12. Not use tobacco, recreational drugs, or be on any potentially teratogenic medication

If no sperm, inadequate numbers of sperm, or very poor quality sperm are obtained, the couple must be willing to use donor semen.  If the couple is considering epididymal or testicular sperm extraction, the male partner must have normal hormone levels, and possibly genetic testing.  If no sperm or poor quality sperm are obtained upon epididymal or testicular extraction, the couple must be willing to use donor sperm to fertilize the retrieved eggs.  The male evaluation and extraction procedure are not included in the IVF Guarantee Program charge, and will be performed, if necessary, at an additional cost.
