A Community Talk on Infertility – Jacksonville

Drs. Winslow, Freeman, Duffy and McCoy will be covering the topics of Infertility Evaluation, Male Infertility, In Vitro Fertilization, and Third Party Reproduction, with opportunities for Q&A. Refreshments provided.

F.I.R.M. will hold a drawing for a FREE CYCLE OF IVF – MUST BE PRESENT TO WIN and have photo ID.

Note: Completion of this form is for RSVP purposes only. Actual entry for free IVF cycle will be at the seminar on March21. Only one IVF drawing entry per couple. This free IVF cycle is not transferable, and must be used by winning couple. Occasionally the Florida Institute for Reproductive Medicine will host fertility seminars in different locations within several weeks of one another. While all are welcome to attend all seminars in all locations, only one entry for the free IVF cycle per couple in any six month time period in any location is allowed. Terms and conditions apply.
