In Vitro Fertilization (IVF)


In Vitro Fertilization is commonly referred to as "IVF".  It is the most widely used form of assisted reproductive technologies (ART) in the world today.  It is basically a combination of injectable gonadotropin hyperstimulation followed by a transvaginal removal of mature eggs.  Those retrieved eggs are then fertilized with sperm in the laboratory, resulting in embryos.  Those embryos grow in the IVF lab for two to five days, and are then placed in the uterus in a simple, pap smear-like fashion.  The exact number of embryos placed in the uterus depends on a number of factors, with patient age, prior success, and embryo quality among the most important.  Any additional embryos created that are not transferred into the uterus may be cryopreserved (frozen) and stored for later use.  IVF consistently produces the highest success rates of any therapy for couples using their own eggs and sperm to achieve pregnancy.

For couples who have a significant male factor as part of their infertility problem, in vitro fertilization combined with intracytoplasmic sperm injection (ICSI) is often their best option for achieving pregnancy.  When IVF with ICSI is performed, the recovered eggs are singly injected with sperm using a microscopic glass needle.  This helps ensure the highest fertilization rates possible.  If a semen sample has a low count, poor movement, or poor shape, ICSI is performed.  ICSI may not be required if the semen sample is normal.
